Note: registration forms for recreational activities are available within our Recreation, Culture and Community Activities Department pages.
Any request to obtain a document must be sent to the Town Clerk’s office by fax, mail, email or in person. The law calls for a 20-day deadline to follow-up on requests but may extend that deadline by 10 days if necessary. A fee is charged to obtain a copy of a document.

Under Section 585 of the Cities and Towns Act, anyone intending to claim damages from the Town shall, within fifteen (15) days after the date of incident, give or cause to be given written notice of such intention, or complete and forward the form provided for that purpose to the town clerk, failing which the Town shall not be required to indemnify the claimant.
